To encourage continuity of care, we ask that you chose a preferred provider during the registration process. Please request medical records to be transferred to us from your previous care provider. Bring your child’s shot record and any important medical history information to your first visit. Bring your insurance card and picture ID to ALL appointments. This is to ensure proper claims processing. Follow up with your provider within 7-14 days of discharge from hospital, emergency room, or urgent care facility visit.
If you plan to be out of town and are leaving your children in the care of a relative or friend, please complete the Medical Proxy Form to ensure your child’s care is not delayed due to legal authority issues.
IMPORTANT: Please bring your child’s current vaccination record with you for the scheduled new patient appointment to ensure we provide the needed vaccines at the appropriate times.
Here at St. Augustine Pediatric Associates, P.A., we are committed to providing the best care for all our patients. To achieve this, we must have a relationship based on mutual trust and respect between us and the parents or guardians of our patients. Immunizations are one of the most important ways of protecting our children from serious life threatening diseases. Due to the recent increase in unfounded fears of possible side effects, many families are opting out of vaccinating their children. While we respect the parent’s right to decide whether to vaccinate or not, we, as public health advocates, strongly believe it is our duty to protect both your children and the children in our practice. To do so, we must enforce the requirements mandated by the Florida Department of Health and strongly encourage patients to follow CDC and AAP vaccination standards. If you are opposed to complying with these vaccination standards, we ask that you find another medical practice that may be more suitable for your child’s healthcare.
